Access Your Medical Records Online
Many patients can view parts of their medical record online through the NHS App or other approved online services.
If you're in the UK and want access to your retrospective (historical) GP medical records, you can request them from your GP practice after setting up the NHS App or other approved online service. Ask at reception for the Online Access to Retrospective Health Records Form
What You Can See Online
Depending on the services available at your practice, you may be able to view:
- Current medications
- Allergies and adverse reactions
- Test results
- Immunisations and vaccinations
- Consultations and clinical notes
- Letters and documents held in your record
How to Register for Online Access
You can access your records through:
- The NHS App ( NHS app or NHS website. )
- NHS.uk online services
- Other approved patient access systems used by the practice
To register, you may need to:
- Provide proof of identity.
- Confirm your contact details.
- Complete any required registration forms.
If you need assistance setting up online access, please contact the practice reception team.
Contact your GP practice and ask for:
- "Prospective access" (new records going forward), or
- "Retrospective access" (historical records already held by the practice). Ask at reception for the Online Access to Retrospective Health Records Form
Access for Children and Other Individuals
Parents, guardians, carers, and individuals acting on behalf of another person may be able to request proxy access where appropriate. Additional identity and consent checks will be required.

Requesting a Copy of Your Medical Records (Subject Access Request)
Under UK data protection legislation, you have the right to request access to personal information that we hold about you. This is known as a Subject Access Request (SAR).
What Information Can I Request?
You can request:
- A copy of your medical records
- Specific information contained within your records
- Information about how your personal data is processed
How to Make a Subject Access Request
You can submit a request by:
- Completing a Subject Access Request form (available at reception)
- Writing to the practice
- Emailing the practice
Please provide:
- Your full name
- Date of birth
- Current address
- Contact details
- Details of the information you are requesting
We may ask for proof of identity before processing your request.
How Long Will It Take?
We aim to respond to Subject Access Requests within one calendar month of receiving:
- A valid request
- Any required identification documents
In some circumstances, where requests are complex or extensive, the response period may be extended in accordance with data protection legislation.
Is There a Charge?
Subject Access Requests are usually provided free of charge.
However, a reasonable fee may be charged for excessive, repetitive, or unfounded requests, as permitted by data protection law.
How Will I Receive My Information?
Information can be provided:
- Electronically via secure email
- Through a secure online method
- As printed copies where appropriate
Please indicate your preferred method when making your request.
Third-Party Requests
If someone is requesting information on your behalf, we will require:
- Your written consent, or
- Evidence of legal authority to act on your behalf
Additional verification checks may be required.
